Membership Application

I, the undersigned Applicant, do hereby request membership in Hi-Speed Track Club for the current season.

I have current membership in AAU and USATF. I agree to abide by the rules of Amateur Athletic Union
(AAU), USA Track and Field (USATF), Hi-Speed Track Club (HSTC), and there affiliate
organizations, and sponsors. Recognizing the possibility of physical injury, and/or death,
associated with Cross Country, Track and Field, Soccer, and other activities; and in
consideration of such membership, the right to participate in training, and competitive events; I,
on my own behalf, hereby release, discharge and/or otherwise indemnify Hi-Speed Track
Club and Hi-Speed Track Club Foundation of Los Angeles, its officers, directors, employees,
staff, affiliate organizations, sponsors, consultants, volunteers and associated personnel of the
aforementioned entities, including owners of the facilities and fields that are utilized by Hi-
Speed Track Club, from all liability by or on behalf of the applicant-registrant, as a result of my
participation in Hi-Speed Track Club activities, and/or being transported to or from the same,
which transportation I hereby authorize.
